Strange ...  so what are the details of your setup: OS version/architecture?
When you open the helpfile manually, you can browse through it without any issue?
Running this commandline command doesn't work either?:  "c:\Program Files (x86)\AutoIt3\AutoIt3Help.exe"

Browsing through the helpfile manually is working without problems.

I am running Windows 10, 1909, x64 with all current updates installed. However this is a Windows installation where the MsMg toolkit has been used, to remove Windows components. Maybe this is causing the issue -  I should have thought about that in the first place. Are the any requirements for AutoIt3Help or system files that should be there?

I did ran some tests andfound the issue.

One removed component was the Internet Explorer and apparently that breaks the HtmlHelpA functionality
